Just print them out, complete the forms and bring with you to your first appointment along with your doctor's referral (if applicable) and your insurance cards and/or a method of payment.

Picture above shows the clinic from the patient parking lot.
The entrance to the patient parking lot is on the left side of Mechem when you are driving NORTH from Sudderth Dr.
Just past the driveway will be our new BLUE ENTER sign. Turn left into our driveway in front of the sign. If you pass Mark Condos, you've gone too far!
Picture above shows the clinic when driving SOUTH on Mechem as you are passing Landlocked Restaurant. To get to the patient parking lot, just keep driving past the clinic until you see the new BLUE sign in the pine trees.
The sign is JUST before the driveway, turn right just past the sign and drive up into the patient parking lot.
